My Lords

This is the fourth application to respecting the ill treatment which I have received from the parish of [Llan] and township of Bala.  Having served in the navy as surgeon during the reign of his late Majesty George III and the Regency of George IV, I left Bala about the 23rd May [there] having [one] payment due at 1 shilling 3 pence a week. I did expect to receive my parish allowance on my return.

However, to my surprise they rejected paying me. I, therefore finding no redress [through] your Lordships’ interference, shall apply to both Houses of Parliament for the same [reason]. I remain your Lordships, obedient and humble servant.
