Present Illness – about the middle of May he began to suffer from pains in head and sore throat. He was taken to the F.A. [Field Ambulance] where he improved greatly and after some days was sent back to duty. After 2 days in the trenches he began to suffer from pains in the legs spreading up to the hips and across the back. In addition he had severe abdominal pains. His appetite was poor and his bowels constipated. At the base he improved greatly.

Present condition – he feels very comfortable apart from slight pains in legs at night. Appetite very good. Bowels fairly regular. Urine normal.

Circulation + Respiratory System – Negative
