- Certificate holder's last name when the certificate was issued
- Certificate holder's country of birth
- Certificate holder's year of birth - add the month and day if known
- Country where the certificate was issued
- Year when the certificate was issued (either an exact year, or a range of years) - add the month and day if known
- Certificate number if known
