1st. Certificate of Age and Enlistment.
THAT William Affleck born in the Parish of Saint Kitts in or near the Town of West Indies in the County of     was enlisted for the aforesaid Regiment at Hounslow in the County of Middlesex on the 28th. Day of July 1801. at the Age of Twenty for Unlimited Service
2nd. Certificate of Service. THAT he hath served in the Army for the space of Nineteen Years and      204 Days, after the Age of Eighteen, according to the subjoined

3rd. Certificate of the Cause of Discharge. THAT in consequence of the loss of his front Teeth, he is rendered incapable of performing the duty of a Trumpeter, he is also subject to pain of the chest, since exposed to wet & cold in service in France in 1815.
HE IS HEREBY DISCHARGED.

4th. Certificate of not being disqualified for Pension. THAT he is not, to my knowledge, incapacitated by the Sentence of a General Court Martial, from receiving Pension.
5th. Certificate of Character, &c.&c.&c. THAT his General Conduct as a Soldier has been good   Served in the Campaigns in 1808. 9. 13. 14 & 15. - I beg to recommend him for your consideration.
6th. Certificate of the Settlement of all Demands. THAT he has received all just Demands of Pay, Clothing, &c. from his Entry into the Service to the date of this Discharge, as appears by his Receipt underneath.
7th. Acknowledgement of the Receipt of all Demands. I William Affleck do hereby acknowledge that I have received all my Clothing, Pay, Arrears of Pay, and all just Demands whatsoever, from the time of my Entry into the Service to the date of this Discharge.

TO prevent any improper use being made of this Discharge, by its falling into other Hands, the following is a Description of the said William Affleck He is about thirty seven Years of Age, is five Feet 9 1/4 Inches in height, black Hair, black Eyes, black Complexion and by Trade or Occupation a Labourer
