THAT in consequence of the loss
of his front Teeth, he is rendered incapable of performing the
duty of a Trumpeter, he is also subject to pain of the chest,
since exposed to wet & cold in service in France in 1815. |

incapacitated by the Sentence of a General Court Martial, from

Campaigns in 1808. 9. 13. 14 & 15. - I beg to recommend
him for your consideration. |

THAT he has received all just Demands
of Pay, Clothing, &c. from his Entry into the Service to
the date of this Discharge, as appears by his Receipt underneath. |
| 7th. Acknowledgement of the Receipt of all Demands. |
